Home and Community Care Program

The Kanesatà:ke Home and Community Care Program (KHCCP) staffs 3 professional Community Health Nurses; two full-time, one part-time. The KHCCP delivers program elements including professional nursing, in-home respite, medical equipment/supplies and specialized pharmaceuticals. Service delivery is provided to eligible community members requiring homecare with a medical prescription for all of the following:

  • Ongoing assistance with ADL to remain independent
  • Post-op dressings
  • Suture/staple removal
  • Lab/blood tests
  • IM and S.C. Injections
  • Monitoring of P.O. Medications
  • Client/family teaching,
  • Loan of/or assistance in obtaining medical equipment
  • in-home respite

Clients whose needs exceed the scope of nursing service in this area are referred back to CLSC Jean-Olivier Chenier or to a facility which can meet their needs. The Home Care team can be reached by phone at: (450) 479-1123.

The team includes our Elder’s Community Worker, Jocelyn “Kitty” Bonspille, who is indispensable.
